Student Internship Group Insurance

 
Eligibility

The incident must occur during the internship period of an internship course taken in the junior year of college.

Coverage

Any external, sudden accidents not caused by illness (24-hour coverage).

Examples include natural disasters, traffic accidents, etc. (Drunk driving and suicide are excluded from coverage).

Coverage details vary each year in accordance with the policy terms.

Application Procedures
  1. Fill out the application form and attach the required supporting documents.
  2. Submit the documents to the Division of Internship and Career Services on the 2nd Floor of Innovation Building.
Supporting Documents
  1. Police Report / Accident Record: Original or copy with a "True Copy" stamp.
  2. Diagnosis Certificate: Original or copy bearing the official hospital/clinic seal.
  3. Medical Receipts: Original or copy bearing the official hospital/clinic seal.
  4. Note: If treated at two or more hospitals or clinics, a separate diagnosis certificate and receipt must be provided for each.
  5. X-Ray Disc: Mandatory if bone fractures or cracks are involved.
  6. Bank Passbook Copy: Photocopy of the student's account cover.
  7. National ID Copy (or Student ID): Photocopy of both sides.
